Under the requirements of the Federal Patient Self-Determination Act, (PSDA), healthcare organizations must distribute the State-mandated brochure Your Right to Make Decisions about Medical Treatment to all in-patients (including residents and clients) as well as members of designated health plans. The brochure approved by the Department of Health Services to reflect California law about healthcare decisions is now available in an attractive, easy to-distribute format in ten languages.

The PSDA brochure consists of 8 illustrated panels printed double sided in green ink on 8.5" by 14" buff-colored paper.

Sold in packets of 100 brochures @ $30 per packet, or a special "Mixed Package" of 8 languages (10 each of Armenian, Cambodian, Chinese, Farsi, Korean, Russian, Tagalog & Vietnamese) also for $30 per packet

Please indicate below the number of brochures you want in each language. English brochures sold only in lots of 100; other languages available in lots of 50; minimum total order of 100 (except "Mixed Package").

Customization of brochure (institutional logo or information) is available on orders of 3000 or more brochures.
